Description
• Identify and qualify prospective patients who could benefit from Carda Health's remote pulmonary care program through various channels, including inbound inquiries, referrals, and outreach efforts • Educate patients on the features, benefits, and value proposition of Carda Health's telehealth services, highlighting the convenience, accessibility, and effectiveness of remote pulmonary care • Conduct thorough assessments of patient needs, medical history, and eligibility criteria to determine suitability for enrollment in the program • Guide patients through the enrollment process, providing personalized support, answering questions, and addressing concerns to ensure a seamless and positive experience • Collaborate closely with internal teams, including patient care coordinators, healthcare providers, and technology specialists, to facilitate smooth transitions and continuity of care for enrolled patients • Utilize CRM systems and enrollment tracking tools to maintain accurate records of patient interactions, enrollment status, and outcomes, and generate regular reports on enrollment metrics and performance • Stay informed about industry trends, regulatory requirements, and competitive landscape in remote pulmonary care to inform enrollment strategies and drive continuous improvement initiatives • Participate in training programs, workshops, and professional development opportunities to enhance knowledge and skills related to patient engagement, enrollment best practices, and telehealth technologies
