“Providing Services to Adults with Intellectual Disabilities”
• Assess individual needs and develop personalized plans to enhance community integration • Facilitate skill-building activities to promote independence and socialization • Coordinate and accompany individuals on community outings and activities • Provide support and assistance in daily living tasks, as needed • Collaborate with families, caregivers, and service providers to ensure effective implementation of plans • Monitor progress and document outcomes according to agency standards
• High school diploma or equivalent • Experience working with individuals with developmental and intellectual disabilities • Knowledge of person-centered planning and community integration principles • Strong communication and interpersonal skills • Ability to work effectively as part of a team • Compassionate and patient demeanor • Ability to maintain strict confidentiality and adhere to ethical guidelines • Valid driver's license and reliable transportation • Experience working in a community-based setting • Knowledge of community resources and services
