Description
• The Home Care Physical Therapist Assistant will provide physical therapy services to older adults within their home/community environment • Communicate with Physical Therapists regarding plans of care and progression of exercises and goals for each patient. • Maintain communication with Coordinator to successfully schedule your patients in a timely manner • Complete notes thoroughly and within a timely manner • Attend company-wide meetings to discuss various company initiatives to continuously improve clinical excellence and patient care.
Requirements
• Connecticut Physical Therapist Assistant License • Medicare Billing and Documentation Experience • Computer Literacy • Team Player Mentality • Liability Insurance • Current CPR Certification • Ability to wear PPE, including masks, face shields, goggles, gloves, and gowns
